Signs the Process May Be Complete
After taking misoprostol, the uterus contracts to expel the pregnancy. Some signs that often accompany a completed process include:
- Heavy bleeding and cramping that gradually decrease over one to two weeks
- Passing of tissue and clots, most often in the first 24 to 48 hours
- Pregnancy symptoms such as nausea and breast tenderness are beginning to fade
- Return to your normal menstrual cycle within four to six weeks
These are general indicators only. Physical signs alone cannot confirm that the abortion is complete.
Signs Something May Be Wrong
Contact a healthcare provider immediately if you experience any of the following warning signs:
- Soaking two or more pads per hour for two consecutive hours
- Fever lasting more than 24 hours, or a new fever that begins days after taking misoprostol
- Foul-smelling vaginal discharge
- Severe abdominal or pelvic pain
- Ongoing pregnancy symptoms
These can indicate an incomplete abortion, infection, or retained tissue, all of which need prompt medical attention.
Why a Follow-Up Is Necessary
A follow-up appointment, which may include a pregnancy test or ultrasound, is standard care after a medical abortion. A healthcare provider can confirm whether additional care is needed. During that visit, your provider may ask about your symptoms, review your medical history, and measure hCG levels to determine whether they are declining as expected.
An ultrasound may be used alongside these results when the provider needs additional information.
